  • The Aspen Dental Savings Plan offers discounts on select services for members. Participation in this plan is subject to separate terms and conditions.

  • Warranty programs on products such as dentures or implants may be available. Specific terms, coverage, and exclusions apply.

6. Privacy and Confidentiality

We are committed to protecting your privacy. Personal and medical information collected during treatment is handled in accordance with HIPAA and other applicable privacy laws.

7. Limitation of Liability

  • Haynes Dental PLLC and The Aspen Group are not liable for any indirect, incidental, or consequential damages arising from the use of our services.

  • Any claims must be made within 90 days of the date of service.
